All diplomas, transcripts, credentials, recommendation letters, fingerprinting, Tuberculin clearance and other required documents must be submitted at applicants’ earliest convenience before hiring.
GENERAL BACKGROUND INFORMATION
You must give complete answers to all questions. If you answer “Yes” to any question, you must list all offenses, and for each conviction provide date of conviction and disposition, regardless of the date or location of occurrence. Conviction of a criminal offense is not a bar to employment in all cases. Each case is considered on its merits. Your answers will be verified with appropriate police records.
Professionally disciplined means the annulment, revocation or suspension of your Teaching Certification or the receipt of a letter of reprimand from an agency, board or commission of state government. Criminal Offense includes all felonies and misdemeanors.
Conviction includes adjudication of guilt, pleas of “nolo contendre” (no contest), and determinations before courts, juries, judges or magistrates which resulted in fines,
sentences or probation. You may omit: minor traffic violations, and offenses committed
before your 18th birthday which were adjudicated in juvenile court under a juvenile
delinquency law.

BACKGROUND AND CRIMINAL HISTORY CHECKS
On request each applicant for employment must submit a Tuberculin Clearance and completed Criminal Background Clearance and his or her fingerprints at each applicant’s expense. We will supply a fingerprint form to each applicant. The fingerprints will be utilized by local, state, and federal law enforcement agencies to research the applicant’s background. Any offer of employment that we extend to an applicant is conditional upon the successful processing of his or her Tuberculin Clearance, fingerprints and the receipt of criminal history and background check results that are acceptable to our school.INSTRUCTION
* Number 16 through 27: list all areas that you hold a valid teaching credentials and/or certificates. Please submit copies of all certificates via email, fax, or by mail.
** Number 28 through 47 are applicants’ education background information. Please submit transcriptions of all colleges attended (and graduate schools, if application) via email, fax, or by mail.
*** Number 48 through 56: Please list name and contact information of two current or previous supervisors as reference. As for applicants that do not have previous working experiences, please list name and contact information of current or previous supervisors of your internship, or professors who are knowledgeable about your professional competence and personal qualifies. In addition, please submit recommendation letters from these two references via email, fax, or by mail.
**** Number 65 through 85: please list your previous work experiences in chronicle order (from the most recent one to the oldest one).
***** Number 91: art media refer to art supplies and/or other media – such as oil paints, watercolor, colored pencils, pastels charcoal, ceramics, glass art, computer-based graphic applications, etc.
Number 92: Art subjects refer to types of themes or objects that you draw and paint to – such as figurative drawing, landscape, transportation, portraiture, still-life, etc.
Number 93: school of art refer to different genre of art – such as realism, abstraction, surrealism, European classical art, Oriental art, etc.
CERTIFICATION AND RELEASE AUTHORIZATION
I certify that all of the statements on this online form made by me are true, complete and correct to the best of my knowledge and belief, and are made in good faith. I understand that any misrepresentation of information shall be sufficient cause for rejecting my application, withdrawing of any offer of employment, or terminating my employment. I hereby authorize any and all of my previous employers and/or supervisors to release any and all of my personnel records, and to respond fully and completely to all questions that officials of Lascaux Academy may ask regarding my prior work history and performance. I will hold such previous employers and/or supervisors harmless of any and all claims that I might otherwise have against them with regard to statements made to the Lascaux Academy. I further authorize Lascaux Academy to investigate my background, now or in the future, to verify the information provided, and I release from liability all persons and/or entities supplying information regarding my background. My submission of this on-line application includes my acknowledgement and certification that the information provided is true and accurate to the best of my knowledge and belief.
